While many lymphomas present with absolute lymphocytosis, some may not. Peripheral blood smear review is a vital tool. There are several morphologic clues that favor reactive versus malignant lymphocytosis.

Immunophenotyping showed clonal B cells expressing FMC7 and strong kappa light chain and not expressing CD5 or CD10; follicular lymphoma was confirmed on a lymph node biopsy. Flow cytometry revealed 3% of events to be monoclonal B cells that were positive for bright CD20, CD5, and bright κ light-chain; they were negative for CD10, CD23, and CD200.
