Sure, a DM could houserule that, but I think if you're going to hurt yourself to keep your rage going, the visual is a barbarian smacking himself in the face or shoulder to keep himself angry. The visual is not the barbarian taking out a knife and carefully cutting himself or pricking his finger. That doesn't scream 'continue the rage' to me.

The tail has some utility regardless but only some. Generally, other kinds of barbarian (ancestral, bear totem) are superior for nearly any purpose.

If a barbarian has DEX 14 AND CON 16, scale mail will be better than being unarmored until level 8 assuming they put both ASI's into CON or DEX. It only kind of makes sense to go unarmored earlier because of fast movement, but barbarians only need to get close enough to hit the enemy. They usually don't zoom around the battlefield.
